This information is necessary for your employment at Mesabi Range Community and Technical College. The following forms will need to becompleted and returned:
- Notice of Intent to Collect Private Data From New Employees: Read, sign and date this form.
- New Employee Information Form: Include a current resume and copy of college transcripts reflecting your credentials (required for all credit course instructors) and be sure to complete both sides.
- Retirement Checklist for New Hires ( Minnesota State Colleges and Universities): Read, complete, sign and date this form.
- Payroll Direct Deposit Authorization (State of Minnesota): Remember to attach a voided check to this form if you are selecting a checking account for your deposits.
- Employee’s Withholding Allowance Certificate (Federal Form W-4)
- Employment Eligibility Verification (Federal Form I-9): Please stop by with your Social Security Card and Driver’s License (or another document from List A or B---see the enclosed list) for us to photocopy and use to complete this form.
Note that as a new hire, our Human Resources department requires a photo copy of your social security card for verification with the Social Security Administration.
Employees of the State of Minnesota are paid by direct deposit. Paycheck advise stubs are available on the internet (they are not mailed). To access the State of Minnesota Employee Self Service page, visit: state.mn.us/employee.
Sign-in using your 8-digit employee ID number as your user ID (this number should be available in a week or two), and the last four digits of your Social Security Number as your password.
To see your paycheck information once your payment has been processed, select “View Paystubs” from this page. For your convenience, I have included a list of pay periods showing the corresponding payment issue dates.
